You will also receive a bill for any pathology or diagnostic services provided. You will receive a separate bill for your surgeon’s services and the services of the anesthesia personnel. Your payment to the Synergy Spine & Orthopedic Surgery Center is for the facility fee only. We accept MasterCard, VISA, Discover, American Express, and CareCredit, as well as payments in cash or by check. Any co-payment is due on the day of your procedure. The costs for ambulatory surgical services are covered by most health care plans except for your deductible and/or co-pay. Please be aware that your appointment will be canceled if you do not have a ride, are not taking public transportation, or do not have prior approval to drive yourself.It is important to have a responsible person stay with you after surgery, including the first night following your procedure.Please ask any questions that you have regarding your post-discharge and recovery care.Our nursing staff will provide you with a copy of your discharge instructions and will review those instructions with you and your significant other(s).We want you to have support persons, but please remember that space is limited and every patient will have others with them as well.If you have an Advanced Directive or legal guardian document, bring a current copy.You will need a responsible adult (18 years or older) to drive you home after your procedure you will NOT be permitted to drive.Bring a method of co-payment as appropriate for your insurance (see Costs and Payments section).Bring your insurance card, prescription card, and picture ID.Bring a list of all the names and dosages of your current medications with you to the center you may bring your medications with you in their original containers.If you wear contact lenses, bring a lens case and your glasses.Do not smoke or drink alcohol 24 hours before your surgery/procedure.Do not eat or drink anything after midnight, including gum or breath mints.Do not wear jewelry leave valuables at home.Wear something comfortable and easy to remove.Notify your physician if you suspect that you could be pregnant.Shower or bathe before coming to the center You may brush your teeth, but do not swallow any water.Do not smoke or drink alcohol 24 hours prior to your surgery/procedure.Bring any documentation/medical records related to your surgery given to you by your physician.Arrive at the center at the time specified during your pre-operative phone call.If you do not receive a call from our staff by the day before your scheduled procedure, please telephone our business office at 586.838.2035.From our clinical personnel to review your health status, provide you with pre-operative instructions, and identify any special needs you may have.From our business office personnel to discuss your insurance and/or payment options.You should receive two calls from our center.
